ACA Exit Meeting Template
The ACA has produced an Exit Meeting Template to assist employers in collecting pertinent information from employees who are leaving their practice.
The Exit Meeting checklist helps employers to collect information on the employee’s experience while working in the practice, their opinions on their job, their manager(s) and the practice itself, the reason for leaving, and any potential improvements that can be made in the workplace.
Member practices should review and adjust the template to suit their practice and the particular circumstances in which the employee is leaving.
